Found during #624. Pipeline states persist across days. Monday complete stays complete forever. Need midnight reset or timestamp-based skip logic.

Timestamp-based staleness check — yesterday\x27s complete/failed states are treated as not_started. Running states >6h old are also reset (crash recovery).
