Gap: 160 open issues across the fleet have zero comments. They are unscoped, unburned, and invisible.

Acceptance Criteria:

  • Every open issue in timmy-home, timmy-config, the-nexus, and hermes-agent has at least one comment OR is closed as duplicate/stale.
  • All zero-comment issues receive one of: acceptance criteria comment, assignment, priority label, or closure with provenance.
  • A summary comment is posted on #480 listing: issues scoped, issues closed, issues still blocked.
