Closes #491

Policy document fulfilling the final acceptance criterion:

7-day threshold for merge-conflicted (unmergeable) PRs
Standard closure procedure with documented rationale
Exceptions: active milestones, explicit time requests, experimental label
Daily check → 48h pester → close workflow

The three target hermes-agent PRs (#129, #108, #107) were already closed
with triage comments before this work began. This policy formalizes how such
cases are handled going forward so the backlog stays mergeable.

This commit adds docs/STALE_PR_POLICY.md specifying:

- 7-day threshold for merge-conflicted PRs
- Standard closure procedure with rationale comment
- Exceptions for active milestones, explicit requests, experimental work
