## Description

Create ExecutionRegistry for clean command/tool routing, replacing model-decided routing.

- [ ] `command(name) -> CommandHandler` lookup
- [ ] `tool(name) -> ToolHandler` lookup

- [ ] Permission context integration
- [ ] Tests pass
