Closes #1120

Persists bounded delivery health per device/channel, records provider outcomes without blocking healthy siblings, exposes health in Device Setup, and adds a real provider-path test notification with actionable mobile recovery states.

Verification:

  • pytest tests/ -q — 2133 passed, 8 skipped
  • rendered at 320×568 and 390×844 — 44px test control, degraded/recovery states confirmed
