Closes #1214

Give each exported work item a stable iCalendar UID across replans and emit the plan revision as SEQUENCE, allowing calendar clients to update blocks instead of duplicating them. Issue and pull identities remain distinct.

Tests: python3 -m pytest tests/ -q (2299 passed, 11 skipped); packaged mobile Week Ahead journey (4 passed).
