Closes #551

What changed

  • lease-fence unread Web Push polling across app workers
  • isolate transient/expired endpoint failures and bound every send with a deadline
  • move push SQLite operations off the event loop and protect state permissions
  • expose lease/deadline tuning and document operations

Verification

  • pytest -q tests/test_push_notifications.py — 11 passed
  • pytest -q — 1143 passed
  • git diff --check — clean
