docs: fix MCP install commands — use uv, not bare pip

The standard install already includes MCP via .[all]. For users who
need to add it separately, the correct command is:
  cd ~/.hermes/hermes-agent && uv pip install -e ".[mcp]"

The venv is created by uv, so bare 'pip' isn't available. All four
occurrences across 3 docs pages updated.
