Add crisis_hook.py module and integration tests.

agent/crisis_hook.py:
- CrisisSeverity enum (NONE, MODERATE, HIGH, CRITICAL)
- CrisisDetection dataclass
- detect_crisis() for pattern matching
- get_crisis_response() with 988 Lifeline info
- format_crisis_notification() for human alerts

tests/test_crisis_hook_integration.py (29 tests):
- Crisis detection returns correct structure
- 988 Lifeline info in response
- Pattern coverage for crisis messages
- Edge cases (empty, None, unicode, long messages)
- Integration flow matching run_agent.py usage
