fix: close orphaned button tags in index.html HUD

Remove orphan <button> for atlas-toggle-btn (line 155) that was opened
but never closed and had no content. Add missing </button> for
soul-toggle-btn and mode-toggle-btn. Fixes broken HTML nesting.
