feat: unified compile_all.py — produces MD, EPUB, PDF, HTML, manifest

- Single script compiles all distributable formats
- Generates build manifest with SHA256 checksums
- Generates chapters.json for web reader
- reportlab fallback for PDF (no system deps needed)
- QR codes linking to soundtrack, game, website, source
- Closes #18
