Closes #625

What changed

stage replacements without committing metadata until old attachment cleanup succeeds
restore staged/removed attachments when either cleanup or metadata persistence fails
keep discarded Drafts visible and resumable on cleanup failure
add behavioral fault-injection coverage for replacement, discard, and metadata write failures

Verification

pytest -q tests/test_unfiled_captures.py — 11 passed
pytest -q — 1277 passed
