Closes #729

Fetches assigned-issue pages concurrently under a four-page cap and five-second aggregate deadline, then rejects pagination churn, missing IDs, and duplicate IDs before deadline reminder fan-out.

Tests: python3 -m pytest tests/ -q (1413 passed).
