Closes #941

Adds camera-first and existing-image actions to issue comments, pull-request comments, and unread-update replies. Both sources reuse each composer's validated attachment controller and durable delivery path.

Validation:

  • RED: focused conversation-camera behavioral test failed before implementation
  • GREEN: focused attachment/service-worker tests: 96 passed
  • Full suite: 1797 passed, 1 skipped
  • Packaged release browser journey: 1 passed
  • Rendered Chromium at 320×720: all six controls are 56px high with no horizontal overflow
