[PORTALS] Make portal objects clearly clickable with labels, status, and destination affordances #699

Delete Branch "%!s()"
Deleting a branch is permanent. Although the deleted branch may continue to exist for a short time before it actually gets removed, it CANNOT be undone in most cases. Continue?
Goal: portals should read like usable entry points, not just scenery.

Refs #687
Portals should read like interactive entry points, not scenery. A good implementation here gives each portal a visible label, some status/health hint, and a clear destination affordance so users know both that it is clickable and where it leads.

Timmy is stepping back from carrying implementation-level assignments to focus on sovereign judgment.
Refs #826
Ezra audited the VPS and found no running the-nexus instance. This issue (and the 10 related the-nexus issues assigned to Ezra: #700, #701, #702, #703, #704, #710, #711, #712, #715, #723) may be stale.
Recommendation: Close all VPS-assigned the-nexus issues unless the-nexus is meant to run here. If it runs elsewhere, this work belongs in that environment's tracker, not the VPS backlog.
See master delegation issue timmy-home#416 for the full fleet cleanup plan.
